New Patients

When you come to Skinjectables Cosmetic Clinic® for your initial consultation, your face and skin will be examined so that a treatment plan can be customized to specifically meet your needs (such as the type of injectable product you would benefit from, the amount of product you require, the location(s) to be injected).

Immediately following your initial consultation, you can opt to receive the recommended treatment. If you would like to proceed with a procedure, you will be asked to complete the following forms . . .

Your First Treatment

Your expectations and desired outcome will be discussed, as will any concerns you have, to ensure your comfort. Your medical history, allergies, current medications, risk factors, past experience with injectables, and use of vitamins, herbal supplements, alcohol, tobacco, and drugs will be reviewed. Your photograph will be taken for your medical record.

Patients should discontinue Aspirin, Vitamin E, or Ibuprofen /Motrin/NSAID use for at least 1 week prior to minimize bruising. Patients on any blood thinners (coumadin, Plavix, etc.) are at risk for significant bleeding or bruising. These should be discontinued prior to injection but only with consent of your Physician.

Existing Patients

Existing patients must fill out a registration form, if a year has lapsed since their previous procedure. Existing patients must fill out a consent form for each procedure they undergo. Please complete a consent form, print it out, and bring it with you to speed up the process.
